What is Social Prescribing?
Social prescribing is a way for local agencies to refer people to a Community Connector.
Community Connectors give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community resources and services for practical and emotional support.
How can this service help you?
Your Community Connector will work with you to develop wellness goals and will provide ongoing support that meets your individual needs and enables you to achieve your goals.
Your Community Connector can support you to gain information, advice, guidance, support, as well as assisting you to access a range of services that meet your needs.
• Support to make positive changes in your life
• Someone to talk to about how you are feeling
• Help with housing, benefits and financial issues
• Physical activity and recreation
• Art, music and creative activities
• Access specialist services and support
• Support to problem solve and work through challenges in your life
Social Prescribing can help if you:
· feel lonely or isolated
· are caring for someone
· are feeling low, stressed or down
· want to get more active
· want to get involved in your local community
· want to make a lifestyle changes for improved health and wellbeing
Who is it for?
How can I access Social Prescribing?
You can request a referral to the Social Prescribing program from your GP, Nurse Practitioner, Home Health, Older Adult Mental Health, Peace Arch Hospital discharge and pre-surgical admission teams, the Specialized Seniors Clinic and Primary Care Network team. We also accept self-referrals.
